1) The structure gives the organization it's form.
2) The followers respond to the structure and the leaders.
3) The leaders (however) determine the ultimate effectiveness of the organization as the character and skills that THEY bring determines the way problems are solved and tasks are accomplished.

The HOW is what the leader's effectiveness comes from.
